There should be 4 seperate documents. I have attached the assignment requirements in a word document and I have attached my unit notes however independent research of relevant case law and legislations has to be done. I recommend choosing scenario 2 as it would be easier to come up with fictional characters and evidence as required. 
In order to do this, you must:
File a Notice of Application for a Suppression Order:
Use Form 82A for civil matters.
Use Form 6-16A for criminal matters.
Support the Application with two affidavits:
Attach two affidavits from relevant parties to support the application.
File a Summons:
Utilise Form 46A for civil matters to outline draft orders you wish the court to make and, if necessary, to summon parties to court.
Utilise Form 6-1D for criminal matters to outline draft orders you wish the court to make and, if necessary, to summon parties to court.
