Service User
John, a 34-year-old male with a history of substance abuse, homelessness, and mental health issues, is a compelling case because of the close relationship between substance abuse and mental illness, as well as housing status. His case illustrates the emerging social determinants of health in the context of vulnerable populations.
Part 1
Instructions: A service plan’s format and contents can vary based on the purpose, agency/organization standards, and case manager role. This template is intentionally general to give you a broad understanding of service plans.
For this Assignment, you may use complete sentences or bullet points to address the Service Plan section (within the boxes). You must use complete sentences for the Analysis section.
Service Plan

In this box, note the service user’s primary concerns to be addressed in the service plan.
4. SMART Goals and Objectives
In this box, write SMART goals and objectives to address the primary concerns. These goals and objectives are developed in collaboration with the service user.
5. Sociocultural Factors Related to Goals and Objectives
In this box, note any sociocultural factors that may affect achievement of the goals and/or objectives.
6. Strengths and Interests
In this box, identify 2–3 strengths that the service user possesses and/or interests relevant to planning.
7. Plan for Services
In this box, identify appropriate services in the community and begin to plan for them (e.g., mental health services).
8. Referral System
In this box, include more details or action steps about connecting to the services (e.g., the service user will have an appointment on X day, or the practitioner will call Dr. X).
9. Length of Service and Follow-Up
In this box, anticipate the length of time needed (e.g., 5 weeks) to meet the overall service plan goals and determine what kind of case management follow-up will be required.
Part 2
Write a 1-page analysis addressing the following:
Justify your plan, based on your service user, their strengths, and their needs.
Explain how to ensure that the plan is person-centered. (That is, how would you empower the service user to engage in the planning instead of relying on you for answers? How would you ensure the service user has choice and dignity?)
Assess what is missing from your plan, based on gaps in the case study. List 2–3 further questions you would ask to get the needed information. Then, explain how you would engage the service user if they only answer the questions with “I don’t know.”
