approval info: The purpose of this project is to include a method of continuous
physical therapy services, known as telehealth services, to ensure
uninterrupted physical therapy attendance.
Improve productivity of staff to patient visit, and Improving patient
compliance with physical therapy services to improve patient functional
outcomes.
However,
the project involves the process improvement of the practice at the practicum
site
Plan: The project is to bridge the gap of no-shows, missed appointments,
and cancellations of physical therapy appointments after surgeon referral. The
purpose serves as improvement patient compliance with therapy appointments and
functional outcomes.
Pre-intervention will include data collection and record review of the number of
no-shows and cancellations of appointments/patient schedules and number of
visits per week/Compliance with the telerehabilitation process
Intervention activities: Education Observation, Record review
post-intervention will involve comparing the changes in practice based
on the implementation and the evaluation of the process.

Analysis using PDSA framework (Refer to Project Charter)
The
purpose of this assessment is to explain the data collection methods and instruments for
your project as well as analyze the data to determine the extent to which you
have achieved desired project outcomes. You will be using the feedback from

considered noncompliance and may result in sanctions; typically, the IRB will
not let you use any data collected.
