I need 30 initial visit progress notes and 30 follow up progress notes and 30 progress notes that may indicate substance use. It should be a total of 90 progress notes. I also need 30 case management notes

Please identify what notes are what by placing ea ch section together? Does not need to be long because the office visit is only 25 min.
