Community Assessment Paper: (See Grading Rubric Appendix A attached)
Purpose: To identify one of the health topic areas from Healthy People 2030 for the community in which you live or work and analyze the variables that may have an impact on this health indicator. Use the rubric (See Appendix B) to help guide you in creating your community presentation.
Define the community you have been assigned to. Include the geographic location & boundaries. (5%)
Describe the community. You may need to use a variety of sources to complete sections of the guide. You may find the following sources helpful: www.nyc.gov, data from local community boards, planning boards, department of health, census data and locally updated information sources. 20%
What types of housing are represented? (Single Family Homes, Public Housing Units, Condos, etc.)
What access do the residents have to shopping? (e.g., Grocery Stores, Pharmacies, Other)
What forms of public transportation are available? (e.g., nearest bus stop, subway station)
What parks/recreational areas are available?
What significant enclaves or discrete populations are represented? (Senior Citizens, Middle-Aged Adults, Children, Ethnic Group)
What religious organizations or churches are represented?
What social agencies, schools, or health facilities are nearby?
What obvious health hazards are noticeable? (Busy Streets, Lack of Sidewalks, Lack of Traffic Lights, Poor Lighting, etc.)
Based on your community assessment, select one health topic area from Healthy People 2030. Give your rationale for choosing the health topic for your area. (20%)
What resources are currently available in the community to assist the residents with the problem you selected from Healthy People 2030 (health topic area)? (20%)
What would you propose as solutions to resolving the health problem (health topic area)? Discuss the role/responsibility of each of the following in the resolution of the problem: the government, families, individuals, health systems, and educational facilities. (25%)
Format (grammar, punctuation, spelling, and proper use of citations, references, and APA format). (10%)
This paper draws on information collected from Internet searches or local newspapers. Demographic information must be from legitimate sources with citations of the source. It is not expected to be a lengthy paper (no more than 10 pages). Please be specific and do not be repetitive.
