As quoted from the syllabus, a course outcome will be:“Given clinical data from an actual evaluation or simulated case study, score and interpret results from standardized and non-­standardized tests and develop an evaluation report that includes:
A summary of the case history information
A description and summary of performance on the various tests.
A diagnosis, including, when appropriate, severity of the disorder, cause, and description of the salient characteristics.
A prognosis with supporting rationale.
Discussion of appropriate management strategies.
Referrals for additional services when appropriate.”
The above outcome will be demonstrated by completion of a paper. This is a report describing a fictional visit with a fictional patient with a hearing disorder (NOT a patient with normal hearing).
Submit a report of a fictional patient encounter and include an audiogram graph.  The report should be written in full sentences and paragraphs.  There will be a possible total of 30 points. Five points are possible for each of the following:
Statement of the problem.
Case History, including symptoms and relevant related health problems.  
Description of otoscopy and test results, including a copy of the audiogram with appropriate and proper recording of test results using correct symbols.
Selection of appropriate test battery for the patient’s age, abilities, and hearing disorder
Conclusions and plan, with adherence to proper scope of practice (audiologists are not physicians or surgeons).
Professional tone of the report, including proper syntax, spelling and notation of all references used.
The written narrative is to include all sections of the SOAP format (the letters SOAP do not need to be included).  
The case study must be written in a professional manner without abbreviations or acronyms
At a minimum, the report should include your textbook as one resource, at least one current peer-reviewed journal article as a second resource, and at least one website associated with the disorder as a third resource/reference.
The case study write up should not exceed three double-spaced pages, not including the audiogram.  The audiogram you plot out and submit will be based on an audiogram provided from a resource. You are not creating your own audiogram data; rather, you will use an audiogram from the resource and plot that audiogram out on the UCF form to submit.  Do not copy and paste an image of an audiogram.  Plot the audiogram yourself using data from your resource. You must include the reference(s) for the source(s) from which the audiogram data was found.  If the journal article does not provide you with an audiogram, use “Google Images” to find an audiogram associated with the disorder.  You must provide that Google link as a fourth reference/resource.
Use the audiogram from the link below. You may print it, write on it and either scan it in (preferred) or include it as a picture.  Values for the acoustic reflex and the OAE spaces may be entered as simply P (present) or A (absent); actual numeric values are beyond the scope of this class.  Likewise, tympanogram type will be sufficient; numeric values will not be needed.  Fill in the top part of the audiogram (from reliability through referral reason).  Unless there is a reason stated in the narrative, pure tone averages (PTA) and speech recognition or awareness thresholds (SRT or SAT) should be included.  Word recognition scores may be noted as simply “good” or “poor.”
Remember to plot air and bone conduction thresholds.  Include proper masked symbols for thresholds obtained with masking.  Remember that thresholds obtained from sound field testing such as VRA will be plotted as “S” rather than “X” or “O.”
If any tests are not done, mark “did not test” or “DNT” in the proper field on the graph and explain in the report why the tests were not done.
Please do not submit your report in .pages format.
