This semester you will be creating an Advanced Service Plan, where you will practice the skills learned in this course. The assignment is divided in 4 modules where you will be submitting a specific part of your advanced service plan. Attached below, you will find a service plan example to help and guide you through the process of completing your own service plan. Please note that the example attached contains the complete service plan, however, for this module you are only required to submit the following part:  
For this assignment, you will choose a family member or friend to be your practice “client” and implement some of the key processes of case management with them this semester.
Part 3: Coordinate services with community resources to help your client and use proper documentation through at least three progress notes. Discharge your client either through referral or successful discharge and end services using the skills learned in class. Please refer to the Service Plan Example for required information.
