Health needs in the community may differ based on the location and the population. Health interventions and/or initiatives often target specific priority populations that may benefit from performing initiatives to help support a change in their health behaviors.
Assignment Directions:
For the Unit 5 Assignment, please do the following:
Choose a health behavior in your community that needs to be addressed (substance use disorder, smoking, impaired driving, etc.).
Complete the provided template.
Briefly describe and differentiate between the theories and models used to change health behaviors
Apply each model to the health behavior you have chosen to address.
